Purchase Orders
We accept purchase orders from Schools, Hospitals, Clinics, Government Agencies and other business organizations that use Purchase Orders.
Please fax your purchase order to 1-888-206-3335, the purchase order form must have your company's name as the Letterhead and have an authorized signature.
We will review and verify your order information. Once it's approved, we'll ship your order and fax an invoice with the tracking number to your company to request payment .
Sales Tax of 6% will be charged to recipients in Florida only. If your FL organization is tax exempt, please include a copy of your exempt status document along with your purchase order.
